Note:  Formatting does not matter.  This sample is Spartan for simplicity.  Add your own company logo, etc.  For complicated, hierarchical accounts, add text and formatting to help the user understand the data.  Alternatively, make some of the fields hidden so the user will have fewer choices.

If you display this on your web site, you are done.  The user's browser will add his name and password.  Alternatively, your server can send this data to our server automatically, without involving the user.  Contact Trade-Ideas for more information.
